As a Christian parent, I affirm that I understand my role as the primary teacher of the faith to my child. I have read the guidelines for preparation for and reception of the Sacraments. I understand that readiness to receive the Sacraments is not determined by grade-level or earned by completion of any particular program but that it depends upon the readiness of my child as demonstrated by a sincere practice of the faith, sufficient catechesis, and a proper disposition to receiving the Sacrament. I promise to support my child’s spiritual growth by practicing the faith and participating in our parish’s Family Faith Formation program and/or Youth Group.
